SourcesWe collect consumer health data directly from you, and in limited circumstances from a treating provider you direct to communicate with us.
PurposesWe use consumer health data only for the purposes of responding to your inquiries, processing payments you initiated with FSA/HSA accounts, maintaining required records, and complying with law.
SharingWe share consumer health data only with the recipients described in the Privacy Policy. We do not sell consumer health data. We do not share consumer health data for advertising or profiling purposes.
Your RightsRight to confirm whether we are collecting, sharing, or selling your consumer health data, and to access it;Right to withdraw consent to our collection and sharing of your consumer health data;Right to have your consumer health data deleted, and to have our processors and certain affiliates and third parties delete it; andRight to appeal a denial of a rights request.
How to Exercise Your RightsSubmit a request at MHMDA@sqairz.com. We do not require account creation to submit a request. We will respond within 45 days, with one 45-day extension where reasonably necessary, consistent with MHMDA.
Consent and AuthorizationWhere MHMDA requires consent for collection or sharing of your consumer health data beyond what is necessary to provide a product or service you have requested, we will obtain that consent through a separate, clearly presented prompt before the collection or sharing occurs. We do not sell consumer health data; if our practices change, we will obtain valid written authorization meeting the requirements of MHMDA § 19.373.040 before any sale.
